I'm a Nurse Practitioner based in rural Virginia with over a decade of experience in family medicine. My mission with Vitality Care is to redefine healthcare for adults 18+ who want to be the best version of themselves.

No. A direct primary care model is a membership arrangement where patients pay a recurring fee for access to defined primary care services. DPC is NOT health insurance and covers only the services included in the membership agreement. It is recommended that all members maintain at least a catastrophic or high deductible plan for emergencies, surgeries and other outpatient treatment services.

Any adult > 18 years of age is welcome to join. However, Telemedicine is not right for everyone. Vitality Care isn't the right fit for high-risk chronic disease management that needs hands-on evaluation — brittle diabetes, active cardiovascular disease, congestive heart failure, or active cancer care. Vitality Care also does not do chronic pain management (including the long-term prescribing of controlled substances), workers' compensation injury evaluations, formal disability determinations, Department of Motor Vehicles (DMV) medical evaluations, and commercial driver's license (CDL) physicals.

Vitality Care does not accept insurance for provider visits — that's how the membership keeps care predictable and unhurried. Labs are not covered by DPC and are an additional charge; patients may personally submit claims to their insurance for possible reimbursement if desired, but reimbursement is not guaranteed.

Virginia only. Vitality Care is a telemedicine-only clinic, and you must be physically present in Virginia at the time of your telehealth visit.

No. Pediatric-specific growth tracking, vaccine management, developmental screening, acute health concerns, and school forms are best suited for hands-on examination by a pediatric or family medicine clinic.

No. Vitality Care is a telemedicine-only clinic.

Direct access to the same provider, with access generally within 48 hours. No office regulations or insurance delays. Vitality Care includes both acute and wellness care under one membership.

Absolutely — prescriptions are part of the visit when appropriate, and sent to your preferred pharmacy.

Depending on your location, laboratory sites can be found by zip code. Vitality Care will help arrange those locations and, in many instances, can provide direct lab testing in your own home. Labs are not covered by DPC and are an additional charge; patients may personally submit claims to their insurance for possible reimbursement if desired, but reimbursement is not guaranteed.

Yes. GLP-1 and dual GIP medications can be prescribed to complement obesity management when clinically appropriate.

Yes and no. Most advertised peptides are not yet FDA approved, but some peptides are approved and available through accredited compounding pharmacies. Select evidence-based supplements are available for individual purchase without a prescription. Hormone therapy is considered after consultation and laboratory testing. Vitality Care is dedicated to providing an individualized treatment plan under shared decision making.

Anti-aging and longevity are broad terms that cover interventions intended to support healthy aging. It is not age reversing. These services can include hormone optimization, metabolic medications, select supplements, labs and biomarker tracking, skin care, lifestyle coaching to include stress management, smoking and alcohol reduction, sleep management, wellness, preventive care, and cancer screening guidance. Recommendations are individualized and determined under shared decision making and evidence-based practices.

Emergencies require in-person evaluation — call 911 or visit your nearest ER. Vitality Care recommends keeping at minimum a catastrophic health plan for hospitalizations and surgeries.

Yes. Many of our members appreciate having both options available.

OptiMantra does not allow patients to cancel auto-pay directly. To cancel services, send a direct message through the patient portal or email [email protected] at least 7 days prior to the first (1st) day of the upcoming calendar month and billing period. No refunds are issued.

Everything is included except for cost of labs, prescriptions, or other diagnostics. There is a $25 dollar "no show" fee and a "Forms" fee of $15 for FMLA or other accommodation requests. There is no fee for work notes.
